PATH (Personal Action Toward Health) Program

PATH (Personal Action Towards Health) is a Chronic Disease Self-Management Program that teaches practical skills for living a healthy life with a chronic health condition like diabetes, arthritis, asthma, chronic pain, or heart disease.   Classes meet for 2 1/2 hours once a week for 6 weeks.

PATH - Diabetes Program

This new program is specifically for adults with Type 2 diabetes and their family and caregivers.   It covers diabetes self-management topics such as healthy eating, physical activity, sick day management, foot care, medications, managing hyper- and hypo- glycemia, and working with health care providers.

Diabetes Prevention

Have you been told you are Pre-diabetic or Borderline Diabetic?   Do you have family members who have Diabetes?   Are you overweight?   This means you may be at high risk for developing Diabetes.   Take action by attending Diabetes Prevention classes!   You will learn about healthy eating, exercise, weight loss, and other ways to help prevent Diabetes.   This 5 week program meets for 1 hour each week, and is taught by 2 trained leaders.

Healthy Eating for Successful Living in Older Adults

Healthy Eating for Successful Living in Older Adults™ (Healthy Eating™) is a program for seniors who want to learn more about nutrition and how lifestyle changes can promote better health.   The focus of this program is to stress heart- and bone-healthy nutrition strategies. These strategies help maintain or improve participants' wellness, independence, and prevent chronic disease development or progression.

Healthy Eating™ is an educational and hands-on program using the revised Food Guide Pyramid (MyPyramid™) as a framework.   In addition, the program includes recommendations and support for physical activity in conjunction with sound nutrition practices.

The Healthy Eating™ workshop is conducted over six (6) sessions.   Sessions are held weekly and run 2 1/2 hours each.   The program ends with a restaurant outing to apply the knowledge and skills learned throughout the course.   Also included in the program is a trip to a local grocery story and a cooking demonstration.

Creating Confident Caregivers

Creating Confident Caregivers is an exciting new program in select regions of Michigan, for caregivers who are caring for a family member living at home with a dementia (such as Alzheimer's disease).   This program is the result of years of research and is designed to reduce the stress of caregiving.   Creating Confident Caregivers highlights the university-tested Savvy Caregiver training program, and empowers caregivers with the key skills, knowledge, and sense of competence needed for "confident caregiving."

Creating Confident Caregivers is available in the Upper Peninsula through a federal grant from the U.S. Administration on Aging to the Michigan Office of Services to the Aging, and the U.P. Area Agency on Aging/UPCAP.   There is no charge for the six weekly two-hour sessions led by trained dementia care specialists.   Participants who successfully complete the workshop may qualify for a cash incentive.   In addition, Respite Care may be available to allow caregivers to attend sessions.

Creating Confident Caregivers for Veterans

Creating Confident Caregivers for Veterans is a special new program designed to address the specific needs of veterans or their family members caring for a loved one with Alzheimers's or dementia at home.   This workship, which highlights the university-tested Savvy Caregiver training program, is held three (3) hours - once a week - for four (4) weeks.   The workshops are led by trained dementia care specialists and provide participants with information, skills, and attitudes to manage stress and increase their effectiveness as a Caregiver.

A Matter of Balance Program

Have you turned down a chance to go out with family or friends because you were concerned about falling?   Have you cut down on a favorite activity because you might fall?   If so, A Matter of Balance: Managing Concerns About Falls is a program for you.

Fear of falling can be just as dangerous as falling itself.   People who develop this fear often limit their activities, which can result in severe physical weakness, making the risk of falling even greater.   Many older adults also experience increased isolation and depression when they limit their interactions with family and friends.   A Matter of Balance can help people improve their quality of life and remain independent.   A Matter of Balance is designed to reduce the fear of falling and increase activity levels among older adults. Participants learn to set realistic goals to increase activity, change their environment to reduce fall risk factors, and learn simple exercises to increase strength and balance.

A Matter of Balance: Managing Concerns About Falls will be offered to Seniors across the Upper Peninsula.   A small fee helps to cover program costs, workbook, and refreshments.   Please call 1-800-338-7227 or dial 2-1-1 to register or for more information.
